This widely used self-report inventory measures adjustment and self-image in adolescents (from 13 to 19 years of age). It gives you an accurate picture of how teenagers think and feel about themselves. Originally developed for use with normal adolescents, the scale is also an effective way to identify troubled teenagers. |

Composed of 129 simple statements, the Offer Self-Image Questionnaire for Adolescents, Revised (OSIQ-R) measures adjustment in 12 areas: |

The test can be completed in approximately 30 minutes. Using a 6-point response scale, the adolescent simply indicates how well each statement describes him or her. The OSIQ-R yields conventional T-scores and validity checks that allow you to quickly determine whether the teenager's responses truly reflect his or her feelings. |

The questionnaire is computer scored, using prepaid Mail-In Answer Sheets, CD, or FAX Service. All of these options give you a full interpretive report. Norms, separated by age and sex, are based on a sample of nearly 1,000 adolescents. The Manual presents OSIQ-R profiles for various groups, including adolescents with single-episode and recurring depression, those at risk for suicide, delinquent youths, and teenagers with eating disorders. |
